A coroner has released his report into the death of a first-time mum and her new-born baby in Waikato.
20 year old Casey Nathan died six hours after giving birth to her son, Kymani, in May 2012.
The infant died two days later in Waikato Hospital.
Garry Evans has called the deaths of Casey and Kymani two of the most tragic to be dealt with by a coroner.
He says they were preventable, and the midwife involved was too inexperienced to deal with a high-risk pregnancy.
Coroner Evans says the pair may have been saved had the midwife taken them to hospital sooner.
