The US financier was found dead in his high security prison cell, while he was awaiting trial on charges of sex with minors.
Officials say he took his own life.
Former US prison warden Cameron Lindsay told Mike Hosking authorities made two big mistakes in their handling of the high-profile offender.
"He shouldn't never have been in a cell with anyone to begin. Subsequent to his suicide watch, he should never have been taken off suicide watch."
He says his alleged victims and their families will never get the justice they were seeking.
"From their perspective, I can't help but feel that they are looking at this and think this guy was able to take the easy way out. The criminal charges against Epstein died when he did."
