Civil Aviation Authority Director Graham Harris joins Heather du Plessis-Allan to talk about the latest report surround the 2015 Fox Glacier Helicopter crash.

The Transport Accident Investigation Commission’s released its final findings into the crash that killed all seven on board, pointing out central reasons such as poor weather conditions, inexperience and a breach of weight limits.

It also found the pilot was not adequately trained and may have misjudged his height because because of the weather, which was not suitable.

Harris says the report is confronting.

"It certainly points out significant weaknesses in our performance in the period leading up to the crash."

Harris says he can not comment on whether the pilot caused the crash, but admits he should not have been flying that day.
