A healthcare provider and a caregiver could face legal proceedings following the death of a disabled man.
The young-man died in a house fire in 2012 when his caregiver left him alone to go and buy groceries.
Deputy Health and Disability Commissioner Rose Wall has today released a report finding the agency and worker in breach of the Code of Consumer's Rights.
She says the caregiver should never have left the young man alone and the agency did not have the correct procedures in place.
"They should have a contract in place and they should have been monitoring the caregiver's performance. They should have ensured that he was sufficiently skilled and trained to undertake the role he was performing."
