UPDATED 3:31pm: An inquest looking at why and how a father was able to killing his two children has wrapped up.

The four day inquest looked into the deaths of Bradley, Ellen and Edward Livingstone.

It heard from about 20 witnesses including neighbours, police and medical staff.

Over the course of the hearing Edward Livingstone was described as being a narcissist, who breached a protection order twice officially.

The inquest also heard how Livingstone had talked to numerous people in the months' leading up the shootings about killings his family.

On the last day of the inquest, Southern police's top cop refuted a claim by the neighbour of Katharine Webb she told police about a death threat from Edward Livingstone.

The inquest into the deaths of Bradley, Ellen and Edward Livingstone is set to wrap up today.

Earlier in the inquest it was heard Ms Webb's neighbour Melanie Foot called police about a conversation she had with Livingstone about thoughts he had of killing his family.

Superintendent Andrew Coster told the inquest there was no knowledge of threats.

He stated the officers would have taken the threats very seriously.

"And the staff spoken to were very clearly of the view that if they received information of that nature they would have recorded it."

Foote was also grilled about whether she was mistaken about telling police of his death threats, but she was adamant she told police about the threat.

Under cross examination from police lawyer, Robin Bates, Mrs Foot was pressed on whether she was mistaken about telling police.

She told the inquest, "I don't think I'm mistaken at all, I do recall speaking to the police about this."

Superintendent Coster has reassured a number of changes have been made to avoid extreme cases of family violence.

He told the inquest when officers are in family violence meetings, they raise the red flag issues and get other agencies to follow up with them.

Edward Livingstone had a number of red flags, including the rape of his wife and breaching a protection order twice.

Officers now have the ability to bring information to meetings in a far more organised fashion.

"Any agency can flag a risk based on the information they've received."

Chief Coroner Deborah Marshall has reserved her findings.
