An inquiry has found a civilian died during an SAS-led raid in Afghanistan in 2010.
But it's found the death was justified under international law.
The Operation Burnham inquiry also found four other people also died, but couldn't determine if those people were civilians or insurgents.
It found New Zealand's intelligence agencies accurately reported the allegations of causalities
But it found the agencies could have done more to ensure the possibility of civilian casualties was considered at an inter-agency level, and reported to ministers.
Defence Minister Ron Mark says the frontline of our Defence Force were let down by senior commanders whose post-operation performance showed serious deficiencies.
He says he will oversee the Government response to ensure lessons are applied, and the relevant structures are fit for purpose.
