An investigative journalist says everything that happened during our deployment in Afghanistan is under question, according to a report that's just been released.
The report on the Provincial Reconstruction Team's deployment to Bamiyan was completed in 2014, but was set aside on the request of a commander - who dubbed it "insufficiently accurate".
NZ Herald reporter David Fisher obtained the report after the intervention of the Chief Ombudsman.
He told Rachel Smalley there are questions over details like equipment that was not good enough, as well as bigger concerns.
"So it's big picture stuff. It says that there was no overall campaign plan - that's the plan that would cover the entire time that we were there."
Fisher said that from what the Defence Force says, this is the only review that's looked at that period.
"And if this review has been shelved, then there's nothing that looks at the entire span of that mission."
The mission spanned 10 years and cost eight lives and $300 million.
