The government has just announced the details of its inquiry into how murderer Phillip Smith fled the country while on a temporary release from prison.
The inquiry will be headed by retired High Court judge John Priestley, and look at Smith's prison release and his departure from the country, the use of different identities by criminals, and the adequacy of information sharing between government departments.
The inquiry is expected to take six months, and its results will be made public late next year.
