An independent review into the response to the February 22 2011 Christchurch earthquake has identified a number of major failings, while commending emergency services.
It finds the Civil Defence emergency management teams were dysfunctional and divided, and there was poor communication between central and local crisis centres.
But overall it found the emergency response worked well.
It makes 108 recommendations, six of them major, with the Government agreeing to adopt four of those.
Civil Defence Minister Chris Tremain says there is room for improvement and lessons to be learnt.
