Criticism for Corrections over coroners' reports

The Department of Corrections is being criticised for not eliminating a known suicide risk at Mt Eden Prison before the death of an alleged HIV sex-predator.

Glenn Mills was found dead in his cell in 2009, on the day he was due to appear in court on multiple charges, including infecting or attempting to infect with a disease.

He was hanging by a sheet from bars on the window.

Coroner Katharine Greig says 10 other prisoners died in similar circumstances at the prison in a 15 year period beginning in 1996.

She says six coroners' reports between 1998 and 2005 recommended the window bars be modified.

Coroner Greig says failure to do so was a serious omission.
