There's been a scathing report into the Fukushima disaster in Japan. It's labelling the 2011 nuclear crisis as "Made in Japan", concluding there were a multitude of errors, wilful negligence and serious deficiencies in response. CNN's Ramy Inocencio says the report also blamed some fundamental issues within the Japanese culture. "Ingrained conventions of Japanese culture, reflexive obedience, as well as a reluctance to question authority - the list went on and on. This isn't a problem though with any one person or group or organisation, they said, but with the Japanese mindset itself."
